Teacher Questionnaire
The school contact will receive the questionnaire login forms with the assessment materials.
All teachers listed on the School and Teacher Questionnaire Tracking Form will receive a Teacher Questionnaire Login Form. The label on the top of Teacher Questionnaire Login Form will indicate the name of the Grade 8/Secondary II teacher to whom the form should be handed. The school contact should distribute the login form(s) to them as soon as the forms are received. No substitutions are allowed.
Instruct the teacher(s) that the questionnaire takes approximately 30–45 minutes to complete and should be completed online, using the information and login credentials found on the login form. The Teacher Questionnaire may be completed anytime but must be submitted no later than the date of the last testing session at the school. Indicate that the questionnaire(s) have been completed on the School and Teacher Questionnaire Tracking Form once the teacher(s) have confirmed its completion.
